Medical Student Performance Evaluation Letter (MSPE) Request

LSOM Graduates ONLY:
1. MSPE Payment
- A $15 fee is required per MSPE request.
- Processing will begin only after payment is received.

- Path: School of Medicine > Medical Student Performance Evaluation Letter > View Category
- Note: Transactions above $20 will incur a fee. To prevent this, we recommend submitting each request separately
2. Medical School Transcript Request (If applicable)
- Access the UTHSCSA Office of the University Registrar’s website: Transcript Requests
- Click the “Request a Transcript Now” (burnt orange tab) to go to the National Student Clearinghouse (NSC) website.
- Complete the transcript request form:
- Select “Order Transcript” and provide your personal information.
- Who are you sending the transcript to? Select “Employer or Other.”
- Processing Details: Choose “ERAS”
- Delivery Method: Select “Electronic”
- Recipient Email: medstudentaffairs@uthscsa.edu
Once steps 1 and (if applicable) 2 are completed, please email Sahar Zaeri at zaeri@uthscsa.edu with:
- Purpose of the request(s) (e.g., ERAS, MIDUS, Medical Board, etc.)
- Any relevant details so we can upload and/or submit the documents on your behalf.
