The institution supports good‑faith efforts to resolve concerns at the lowest appropriate level, while also providing confidential and anonymous reporting options when needed.
When Should I Raise a Concern?
You should raise a concern if you experience or become aware of issues related to:
- Clinical supervision or patient safety
- Work hours or fatigue
- Professionalism or ethical concerns
- Learning environment issues
- Harassment, discrimination, or mistreatment
- Well-being or mental health concerns
- Conflicts of interest that interfere with resolution
How to Raise a Concern
UT Health San Antonio encourages a stepwise approach when raising concerns.
Step 1: Talk with Your Chief Resident
In many cases, concerns can be resolved informally and promptly by speaking with your Chief Resident.
- If you are rotating outside your specialty, you may work with both the home program and host program Chief Residents.
Step 2: Contact Your Program Director
If your concern is not resolved—or if you believe there may be a conflict of interest—you may contact your Program Director.
- When applicable, Program Directors will coordinate with directors of other involved programs to address concerns raised during off‑service rotations.
Step 3: Contact the Office for Graduate Medical Education
If concerns remain unresolved, or if escalation is needed, residents may contact the Office for Graduate Medical Education (OGME):

Sexual Misconduct and Title IX Reporting
Residents who experience or witness sexual harassment, sexual assault, or sex‑based discrimination are encouraged to report concerns promptly either via online reporting or through the University Police Department. Support and resources are available regardless of reporting choice.

ACGME Reporting
If a resident, fellow or faculty member in an ACGME accredited or recognized program believes his/her concerns still have not been adequately addressed after following the steps above, s/he should contact the ACGME to present his/her concerns https://www.acgme.org/Residents-and-Fellows/Report-an-Issue/Office-of-Complaints/
While program personnel may contact the ACGME at any time, generally, it is most productive to work initially in good faith with the Chief Resident, Program Director, Office for Graduate Medical Education, GME Wellness Team and/or Institutional Offices.
What Happens After I Raise a Concern?
- Concerns are taken seriously and reviewed by appropriate institutional leadership
- The institution seeks resolution that supports educational quality and well-being
- Confidentiality is maintained to the extent possible
- Retaliation against personnel who raise concerns in good faith is prohibited
