Steps for success:
Step One: Know your Residency Coordinator and his/her needs

Your program coordinator is your ADMINISTRATIVE CONTACT and LIAISON for your residency program. Your program coordinator is will be essential in helping you navigate through requirements for beginning your training. Your Coordinator will have program-specific requests for you, that are not detailed on this page.
Step Two: Schedule a Health Screening at the University Hospital Clinic
In order for residents to start training on July 1st, all residents are required to complete a health screening at the University Hospital Employee Health Clinic located on the 3rd floor.
Incoming residents will need to bring documentation of the following:
- Tetanus/diphtheria or TDAP immunization within the past 10 years
- 2 doses of the MMR vaccine or proof of immunity to measles (rubeola) and rubella
- A current PPD (within the past 3 months of start date). If you have had a positive PPD, youwill need to provide a copy of a chest X ray report taken after the PPD was positive.
- Completion of the Hepatitis B series and a Hepatitis B titer done after completion of the series.
- Two doses of the Varicella immunization or titer stating immunity or verbalization of having chicken pox as a child.
The clinic will also administer a color blind test and a mask fitting test. It is imperative residents complete the health screening prior to administrative processing. The number to the clinic is 210-358-2277.
Step Three: Complete These Hospital/UT Forms

- Housestaff Welcome Letter
- VA Resident Application - Complete and sign on pages 3 & 4. This is not a password-protected form. If you have problems, try another computer.
- Declaration of Federal Employment - Please complete and sign BOTH 17A and 17B.
- Fingerprint Form for Housestaff - Complete all highlighted areas.
- Forms of ID Accepted for Badge Process - Identifies all approved forms of ID you can use in the fingerprint process; choose 2; must be originals
- CPRS Training - Hands-on Tutorial for the VA's Computerized Patient Record
- CPRS Quiz - 5 Question quiz to complete after tutorial. Must pass before computer codes will be given
- Mandatory Training Modules - Explains where and how to log on to online training
- Fingerprint Request Form for use outside of San Antonio - Use this ONLY for fingerprints done anywhere else than San Antonio

Step Four: Obtain your Physician in Training Permit
You will be contacted by your Residency Coordinator when he/she has obtained your ID number to access the TMB website so that you can apply for your training permit. When you have your ID number, please access the TMB website here:

Step Five: Additional Training Modules - IHI
At UTHSCSA we value both your medical education and the safety of all our patients - and we sponsor a chapter of the IHI Open School, and you are invited to participate in chapter activities here. Further, to ensure that all incoming residents begin training with at least a basic level of knowledge about patient safety and quality, one of the important tasks you’ll need to do is complete the IHI Open School online modules, thereby earning basic certification.

Step Six: Additional Training Modules - Patient Safety
To promote safety and quality of care, three educational modules will be required of all incoming residents. The Knowledge Center will send an e-mail with instructions and a link to access the Knowledge Center website. E-mails from the Knowledge Center will be sent on or after 7/1/2011. Thirty days will be allowed for completion of these modules. To access the website, you will need to use your UTHSCSA domain user ID and password. Your program coordinator can provide you with this information. Upon obtaining access, you will be directed to the 3 modules that need to be completed. These modules include:
- Transmission-based isolation precautions
- Hand hygiene
- Bloodborne pathogens training
Step Seven: Familiarize yourself with this Electronic Medical Records System*
*If you will be rotating at the MARC, please familiarize yourself with the EPIC system in general via the modules at the link below. Then plan to review the modules again, to gain greater familiarity with the appropriate system, prior to your specific rotations.
Only residents that will be rotating at the MARC will need to access the EPIC files. Your program coordinator can tell you if you will be going to the MARC.
To access these files, you will need to download and install UTHSCSA Remote Access (VPN). Software and instructions for Remote Access (VPN) are provided at the link below. You will need to use your UTHSCSA domain user ID and password to access the VPN software and instructions link. Your program coordinator can provide you with this information. When entering your domain user ID and password, be sure to include “uthscsa\” before your user name. (Example: uthscsa\username) Once the VPN software is installed and running on your computer, you will be prompted to provide your domain user ID and password before accessing the Epic training modules.
