Root canals aren’t what they used to be

The words root canal often bring to mind pain and stress, but modern treatment is far different. At UT Dentistry’s Endodontics Clinic, patients find safe, comfortable, evidence-based care focused on relieving pain.

“Many people hear ‘root canal’ and imagine pain, but the truth is, it’s a comfortable, safe and pain-relieving procedure,” said Rachel Kurcz, DDS, a resident endodontist at UT Dentistry. Under the guidance of Nikita Ruparel, DDS, PhD, director of the advanced endodontics program at UT San Antonio’s School of Dentistry, the endodontics clinical team is committed to providing patients with both advanced expertise and compassionate care.

Why root canals are needed

Root canal treatments are performed when something called the pulp inside a tooth becomes infected or irritated. According to Kurcz, cavities and decay are not the only reasons patients may require a root canal. While deep decay that reaches the tooth's pulp is a common reason, treatment may also be needed due to cracks or fractures in the tooth structure, trauma or injury, or stress-related bruxism, which is clenching or grinding.

Overall health influences how patients heal; therefore, personalized treatment plans should be made with that in mind. Patients managing chronic conditions such as cardiovascular disease, diabetes or autoimmune disorders may need tailored care or more visits.

“It’s not just about the tooth, it’s about the patient as a whole,” Kurcz said. “We take into account a patient’s comprehensive medical history to set the scene, so the body has the best chance to successfully heal the infected tooth.”

Kurcz went on to mention that the Endodontics Clinic achieves 80–90% success rates in root canal procedures as a result of their personalized treatment plans, advanced diagnostics and imaging and sophisticated cleaning and sealing techniques.

Prevention and advanced treatment

The best root canal is the one a patient does not need. Kurcz emphasized the importance of regular checkups, which can identify problems early and prevent the need for more complex procedures. This also helps lower costs — and pain — over time.

When treatment is necessary, UT Dentistry sets itself apart with advanced technology and expertise. The team offers retreatment of previously unsuccessful root canals. They also use high-resolution computerized tomography scans, or CT scans, dental microscopes and dental lasers to ensure thorough cleaning and improved comfort.

“We use high-tech imaging and microsurgical tools to minimize guesswork. Your tooth gets the most accurate care possible,” Kurcz said.

In some cases, a tooth may require a minor surgical procedure called an apicoectomy. This microsurgery removes a few millimeters of the root tip to eliminate infection and promote bone healing. It is performed under local anesthesia for patient comfort. Kurcz explained that with current 3D imaging technology, endodontists can better plan precision surgical procedures when needed with a targeted approach to cleaning and sealing the root of the tooth.

“Sometimes a root canal treated tooth needs a little extra help. An apicoectomy procedure is a microsurgery that removes the tip of the root and provides a clean slate for the body to heal,” said Kurcz.

Getting the care you need

“Patients often tell us it was nothing like they expected, much easier than they imagined,” Kurcz said. “If there are complications, we are always there to walk beside you.”

For many people, fear is a barrier to treatment, which makes patient comfort a top priority. Local anesthetic ensures pain-free care, and advanced training helps providers deliver treatment with gentle hands and confidence, supporting patients through every step.

“We don’t want patients to let fear keep them from getting the care they need,” added Ruparel. “With the right team, tools and approach, a root canal doesn’t have to be scary, but can resolve pain and preserve your natural smile, keeping your tooth healthy for years to come.”
