UT Health San Antonio Primary Care Marks a Decade of National Recognition for Patient-Centered Care

For a decade, primary care practices at UT Health San Antonio have earned national recognition as Patient-Centered Medical Homes (PCMH) from the National Committee for Quality Assurance, highlighting a sustained commitment to coordinated, patient-focused care.
The NCQA Patient-Centered Medical Home designation recognizes practices that prioritize strong relationships between patients and their care teams, expanded access to care, coordination across specialties and continuous improvement in health outcomes.
“Achieving Patient-Centered Medical Home recognition for a decade reflects the deep commitment our primary care teams have to delivering the highest level of care for our patients,” said Ramon Cancino, MD, MBA, MS, FAAFP, executive director of the Primary Care Center at UT Health San Antonio. “For patients, this designation means they have a dedicated care team that prioritizes access, coordination and long-term relationships. It represents years of building a model of care where patients are truly known, supported and guided throughout every stage of their health journey.”
A Decade of Patient-Centered Care
The PCMH model places patients at the center of their care, emphasizing collaboration among physicians, advanced practice providers, nurses and care managers to ensure every patient receives coordinated, personalized treatment.
“At UT Health, we strive for excellent patient care,” said Amber Clapper, MD, medical director at UT Health Verde Hills. “Patients need to feel their needs are heard and be able to build a trusting relationship with their provider. Access to same-day appointments, the ability to communicate through MyChart and positive experiences in the clinic all help build that trust.”
Coordinated Care Across the Health System
A key element of the PCMH model is coordination across the entire health care system, from primary care visits to specialty care and hospital services.
“Patient-Centered Medical Homes empower patients to be partners in their own health journey,” said Chandana Tripathy, MD, medical director at UT Health Medical Arts and Research Center. “Primary care teams focus on whole-person care while coordinating with specialists and hospital systems in a timely way. Our teams take responsibility for the entire continuum of care and the outcomes that follow.”
